Responsibilities: **Position Summary:** Incumbent manages the care of the Advance Heart failure and Mechanical Circulatory Support (MCS) patient population across the continuum in collaboration with physicians and other members of the multidisciplinary team. Provides advanced assessment, planning and coordination of care, modification of the treatment plan and monitoring and evaluating outcomes. The Nurse Practitioner, provides routine care and management of acute and chronic complaints. The Nurse Practitioner also functions as an expect clinician, consultant, educator and leader in cardiovascular patient care management. Maintains a system for coordinating the caseload of care throughout episodes on mainly inpatient care, and occasional outpatient care.
