Responsibilities: **Position Summary:** Evaluates patients, plans and implements an effective patient focused physical therapy program utilizing standard and specialized treatment techniques, effectively communicates and documents information regarding patients. Establishes and initiates discharge plans as necessary. Consistently evaluates outcome of care and provides continuous monitoring of patients’ needs making appropriate modifications in the treatment plan.
