Responsibilities: **Position Summary:** The Director, Case Management is responsible for the operational leadership and management of all case management functions at their assigned facility or facilities. The incumbent is accountable for the clinical quality, operational performance, regulatory compliance, and financial management of case management services within the assigned area. This role directs departmental activities to ensure case management services are delivered accurately, efficiently, and in alignment with health system standards, payer requirements, and organizational objectives, supporting safe and effective patient transitions across the continuum of care.
