The role involves triaging patients, facilitating treatment involvement, and coordinating care through phone-based interactions. This position emphasizes administrative functions, crisis support, resource advocacy, and community liaison activities, with minimal face-to-face contact. The ideal candidate is detail-oriented, proficient in Microsoft Office and spreadsheet tools, and comfortable managing a high volume of administrative tasks, chart reviews, and crisis escalations.** **Key Responsibilities:** Conduct mental health triage of patients upon provider referral, assessing psychosocial risk factors, reaction to illness, and coping abilities under supervision. Facilitate patient engagement in treatment and educational activities, supporting behavioral health needs remotely. Serve as a liaison between patients, families, medical staff, and outside agencies, including child and adult protective services, law enforcement, schools, and community organizations. Perform functions related to utilization review and discharge planning in collaboration with licensed behavioral health clinicians and multidisciplinary teams. Assist in crisis intervention and de-escalation, supporting safety and therapeutic environment principles. Support placement coordination and resource advocacy, helping connect patients to community services and ensuring proper care transitions.
