This role ensures efficient coordination of patient care activities, supports case managers and social workers, and assists with documentation, communication, scheduling, appeals, performance improvement initiatives, and data tracking to facilitate timely and effective patient care transitions. **Essential Functions** - Provide administrative support to the SW/Case Management team, including case managers, social workers, and department leadership. - Coordinate and schedule multidisciplinary meetings, patient care conferences, and discharge planning meetings. - Maintain accurate patient case records, logs, and departmental databases in accordance with hospital policies and HIPAA regulations. - Assist with patient discharge planning by coordinating communication with external facilities such as rehabilitation centers, skilled nursing facilities, home health agencies, and insurance providers. - Prepare reports, spreadsheets, and departmental metrics related to case management activities, patient flow, and discharge planning. - Manage incoming calls, emails, and inquiries related to case management services. - Track authorizations, referrals, and follow-up documentation to ensure compliance with regulatory and payer requirements. - Assist in maintaining department policies, procedures, and forms. - Coordinate office operations including supply management, scheduling, and document organization. - Support quality improvement initiatives and audits by organizing documentati
