The Veterans Affairs path

The VA is one of the most reliable H-1B sponsors for physicians, and the path bypasses several common problems:

Cap-exempt: No lottery required. VA medical centers are federal government facilities.
Automatic J-1 waiver: The VA sponsors IGA waivers for its physicians.
Diverse roles: Primary care, specialty care, research, telemedicine — all VA medical centers hire.
Geographic flexibility: VA medical centers exist in all 50 states.

The trade-off: VA salaries are typically 10-30% below private practice. But the stability, immigration support, and excellent benefits (federal employee status, pension, etc.) make it a strong choice for foreign medical graduates.

Academic medicine: another cap-exempt option

University-affiliated hospitals are cap-exempt under INA § 214(g)(5)(A). This includes:
